Accrual policy is set by the employer or an agreement, not by federal law — the FLSA does not require paid vacation, holidays or sick leave at all. State and local paid-sick-leave laws do impose minimums in many places, and those sit underneath whatever a plan says. Check the plan document for the numbers above; this works out the arithmetic, not the entitlement.

How to use the PTO accrual calculator
Start by choosing the sentence that best matches the plan: earn a fraction of an hour for every hour worked, earn a fixed number of hours each pay period, or receive a single annual grant. Then enter the rate in that same unit. The calculator does not infer plan terms from a target such as “two weeks a year.”
The loaded example earns 0.0385 PTO hours for every worked hour. Across 1,040 hours, the engine accrues 40.04 hours. It begins with no carried balance, subtracts 16.00 hours already used, stays below the 160.00-hour cap, and returns 24.04 hours. At an 8-hour workday, that is 3.01 days available.

The displayed rate reaches the common 80-hour target across a 2,080-hour year, but its four decimal places make the engine result 80.08 hours rather than exactly 80.00. The exact fraction would be 80 divided by 2,080. Keep 0.0385 when that is the rate printed in the policy; do not silently replace a contractual number to force a round target.

Next enter the starting balance, hours already taken, and the accrual cap. The engine adds new accrual to the carried balance, limits that raw balance to the cap, and subtracts usage afterward. This fixed order makes the calculation reproducible, but a real ledger may accrue and consume time throughout the period. Use a shorter measurement window when timing changes whether the ceiling was reached.
When the raw balance exceeds the cap, the excess is not banked and is not shown as owed later; within this calculation it stops existing. Accrual normally resumes after the bank falls below the ceiling, but only the plan can establish the precise timing. California’s enforcement guidance expressly distinguishes a reasonable cap from prohibited forfeiture of vested vacation and says accrual resumes below the cap.

What happens when accrual reaches the cap
A cap is a ceiling on the bank, not a date. Suppose the carried balance is 100.00 hours and the employee accrues 40.04 more under the loaded rate. The raw balance reaches 140.04. With a 120.00-hour cap, the engine banks 120.00 and reports 20.04 hours refused by the cap before subtracting any entered use.
If 16.00 hours of use is entered in that same aggregate calculation, the final balance becomes 104.00 hours, or 13.00 8-hour days. The refused 20.04 hours do not reappear merely because the ending balance is below 120.00. They never entered the bank under the calculation’s order, while later accrual can resume once the balance is below the ceiling.

A real plan may post accrual and usage on particular dates. When the employee used the 16.00 hours can determine whether some accrual resumed before the period ended.
The timing caveat matters near a ceiling. If time is used early and accrual posts later, more hours may bank than an aggregate cap-first calculation shows. If accrual posts while the bank is full and leave is used afterward, the refused amount may be accurate. Pay-period-by-pay-period entries are the safer method when a disputed balance depends on sequence.
Why a cap is not use it or lose it
A use-it-or-lose-it rule reaches backward and removes leave already placed in the bank at a deadline. An accrual cap works prospectively: the existing bank remains, but no additional hours enter while the ceiling is met. Once the balance drops below the ceiling, earning ordinarily begins again under the plan’s posting schedule.
Federal law does not create a general right to paid vacation, holidays, or sick leave under the FLSA. Those benefits are usually established by an employer policy, employment agreement, or collective agreement, while separate federal, state, or local leave laws may create particular rights. That is why the rate, cap, carryover, eligibility, and payout terms must come from the governing document rather than a national default.
California supplies a clear state-law wrinkle. Paid vacation is treated as earned wages as labor is performed, vested vacation cannot be forfeited, and unused accrued vacation generally must be paid at separation. The Labor Commissioner permits a reasonable cap because it prevents new accrual rather than taking away an existing balance, and combined PTO plans that cover vacation receive the same treatment.

Other states do not all treat vacation like California, and paid sick leave may be governed differently from vacation even inside the same jurisdiction. A combined PTO bank can inherit vacation rules in some places. Treat the calculator as arithmetic after the applicable policy and law are identified, not as a decision about whether a particular ceiling or forfeiture term is enforceable.
The formula, worked line by line
The engine chooses one accrual equation from the selected basis. Per-hour accrual multiplies the rate by worked hours and projects a full year at 2,080 hours. Per-period accrual multiplies by elapsed pay periods and projects with the selected annual period count. An annual grant uses the entered rate once for both current and projected accrual.
After rounding new accrual to two decimals, the engine adds the nonnegative starting balance to form a raw balance. A positive cap limits that raw balance and records the excess as hours lost to the cap. A cap of zero means uncapped. Hours used are subtracted after the cap, and a negative ending balance remains visible rather than being forced to zero.
Days off are a display conversion rather than another entitlement rule. The ending hours are divided by the entered working-day length, with a minimum divisor of one hour. An employee working 10-hour shifts therefore sees fewer days than an employee working 8-hour shifts with the same bank, even though both own the same number of hours.

The annual projection uses the same 0.0385 rate across 2,080 hours and returns 80.08 hours. The small excess over 80 reflects the entered four-decimal rate.
The figure extends that same per-hour rate to 3,120 worked hours. Raw accrual reaches 120.12 hours, but a 120.00-hour ceiling banks only 120.00 and reports 0.12 refused. The flat line is the defining behavior of a cap: the raw accrual can keep rising mathematically while the owned balance does not.
Treat every result as an estimate and not legal advice. The calculator cannot see waiting periods, eligibility classes, service tiers, maximum carryover, blackout dates, proration for part-time schedules, anniversary resets, separation payout, or the posting order inside a pay period. Confirm those rules in the current plan and check applicable law before treating refused or forfeited hours as final.
