How to use the waist-to-height ratio calculator
Enter a waist measurement and a height in the same unit and the tool returns the ratio between them, the band that ratio falls into, and the waist figure that would sit exactly on the 0.5 boundary for your height. The boundary is always half your height by definition, so at 175 cm it is 87.5 cm and at 160 cm it is 80 cm.
Because the ratio is a number divided by a number of the same kind, the units cancel completely: an 85 cm waist on a 175 cm frame gives 0.49, and the same body measured as a 33.5 inch waist on a 68.9 inch frame gives 0.49 as well. The only way to get a wrong answer is to mix units between the two fields.

The reason this ratio exists alongside BMI is that BMI cannot see where mass sits. Two people of the same height and the same weight score exactly the same BMI whether that mass is muscle across the shoulders or fat around the middle, and it is the central fat that tracks cardiometabolic risk most closely.
NICE guideline NG246 therefore recommends measuring waist-to-height ratio alongside BMI for adults with a BMI under 35, precisely to catch the people whose weight looks unremarkable while their waist does not. Above a BMI of 35 the guideline treats central adiposity as already high, so the extra measurement adds little.

NICE recommends this ratio alongside BMI rather than instead of it, because total mass and where that mass sits are two different measurements.

It is one measurement taken with a tape, and a tape sitting two centimetres higher or lower changes the answer: at a height of 175 cm, every centimetre of waist moves the ratio by about 0.006, so four centimetres of measurement drift can carry someone across a band boundary that means nothing on its own.
Do
- Measure at the midpoint between the lowest rib and the top of the hip bone.
- Enter both figures in the same unit, since the ratio is dimensionless.
- Measure against bare skin with the tape level all the way around.
- Breathe out gently and stand relaxed before reading the tape.
- Repeat the measurement the same way so readings months apart are comparable.
Don't
- Mix centimetres in one field with inches in the other, the one mistake that produces nonsense.
- Read a band change as meaningful when four centimetres of tape drift can cause it.
- Pull the tape tight rather than snug, which shifts the reported band.
- Compare a reading taken over clothing with one taken against bare skin.
- Treat a figure outside the healthy range as a diagnosis rather than a prompt.

The bands and the evidence behind 0.5
The bands the tool reports are the ones NICE sets out in guideline NG246, four ranges with a plain label attached to each.

Below 0.4 is worth mentioning to a clinician rather than celebrating, since it can reflect undernutrition or illness.
The evidence behind that single 0.5 boundary is unusually tidy for a health screening rule: a 2012 systematic review and meta-analysis by Margaret Ashwell, Parminder Gunn and Sigrid Gibson in Obesity Reviews found waist-to-height ratio outperformed both waist circumference and BMI as a screening tool for adult cardiometabolic risk factors.
The comparison that shows why height belongs in the calculation at all: a 90 cm waist gives a ratio of 0.563 on a 160 cm frame and 0.474 on a 190 cm frame. The same tape reading lands in the increased band for one person and the healthy band for the other, which is exactly the information a waist measurement quoted on its own throws away.
Height matters in the other direction too. At 175 cm, a waist of 78 cm gives 0.446 and a waist of 100 cm gives 0.571, so a 22 cm difference around the middle spans two bands while BMI, if the weight happened to match, would report a single number.
The formula, worked line by line
There is only one operation here, and its simplicity is the point. Waist divided by height gives a pure number with no units attached, because the centimetres or inches on the top cancel the centimetres or inches on the bottom.
That is why the same boundary works for a person of 150 cm and a person of 190 cm without a separate table for each, and why the rule can be stated in six words: keep your waist under half your height.
Contrast that with BMI, which is weight in kilograms divided by height in metres squared and therefore carries units of kg per square metre. BMI needs its thresholds calibrated, and NICE lowers them for people of South Asian, Chinese, other Asian, Middle Eastern, Black African and African-Caribbean family background, because the same BMI carries a different risk across populations.
A ratio of two lengths sidesteps a good deal of that scaling problem, which is one of the arguments its proponents make for it.

The waist sits 2.5 cm inside the boundary. Measure exactly the same body in inches and nothing changes: 33.46 divided by 68.90 is still 0.486. Move the waist to 95 cm and the ratio becomes 0.543, in the increased band and 7.5 cm past the boundary; at 105 cm it reaches 0.600 exactly, the point where the high band opens, and at 68 cm it drops to 0.389, below the healthy range.
The honest limits. This is one measurement with a tape and it inherits every weakness of that: at 175 cm, one centimetre of waist moves the ratio by about 0.006, so a tape sitting a couple of centimetres too high or too low, or pulled tight rather than snug, can shift the reported band.

The ratio says nothing about muscle, fitness, blood pressure, blood glucose or family history, all of which a clinician weighs alongside it. It is not valid during pregnancy; for children and young people from age 5, NG246 applies the same rule and the NHS publishes a calculator built for them.
