Exactly 1,000, so multiply grams by 1,000: 0.5 g is 500 mg. Below the milligram the same step repeats — 1 mg is 1,000 micrograms — and that lower step is where a misread prefix becomes a thousandfold overdose rather than a rounding error.
Write quantities under one milligram in micrograms where you can. A number with no decimal point cannot lose one.

The formula, worked line by line
One multiplication by an exact power of ten. There is no constant to choose and no rounding to disclose; the only decisions on this page are about direction and notation, and both of those are decisions a calculator cannot make for you.

Stated that plainly it sounds impossible to get wrong, and it is still the commonest error in the area. The reason is that the units look alike, the prefixes are single letters, and nothing in the result tells you that you went the wrong way — a wrong answer looks exactly like a right one.

Four correct answers, one obviously best. Five hundred milligrams needs no decimal point and no exponent, which is the practical test for whether you have picked the right unit.
Case sensitivity is not pedantry
Unit symbols are defined marks, not abbreviations, and their case is part of the definition. Lower-case mg is a milligram; capital Mg is a megagram, which is a tonne — a billion times larger. In a chemistry context Mg is magnesium instead. There is no reading in which the capital is a harmless variant.

How to use the grams to milligrams converter
Multiply by a thousand. The prefix milli- is defined as exactly one thousandth, so the factor carries no uncertainty and the conversion cannot introduce error. Everything worth saying on this page is about which direction you are going and how the answer should be written down.
Direction is the first thing to get right, because the two directions are a millionfold apart when confused. Grams to milligrams multiplies. Milligrams to grams divides. Reverse them and 0.5 g becomes 0.0005 instead of 500, which is not a near miss in any sense.

The ladder is uniform, which is both its strength and its risk. Microgram, milligram, gram, kilogram — each step is exactly a thousand, so there is only one rule to remember, and a slip at any rung produces exactly the same size of error as a slip at any other. Nothing about the arithmetic warns you when you are on the wrong rung.
Notation carries real weight below the gram. A trailing zero is prohibited on medication documentation because 5.0 mg read with the point missed becomes 50 mg. A leading zero is required because .5 mg read the same way becomes 5 mg. Each is a tenfold error produced by one lost mark on a page.

Read the middle column and the numbers are comfortable. Read the left and they are decimals with leading zeros, which is exactly why doses below a gram are quoted in milligrams.

Choosing the unit that reads safely
There is usually a choice of unit for the same quantity, and they are not equally safe to write. A quarter of a milligram can be written 0.25 mg or 250 mcg. Both are correct. Only one of them survives a bad photocopy, a smudge, or a reader skimming quickly, because only one has no decimal point to lose.

The general rule is to pick the unit that puts the number between one and a thousand. It is the same instinct that makes people say 500 grams rather than 0.5 kilograms, and here it has consequences beyond readability.
The microgram symbol is a separate hazard. Written with the Greek letter mu it can be mistaken for an m, turning micrograms into milligrams — a factor of a thousand. Safety guidance therefore asks for the word microgram or the abbreviation mcg instead, and that recommendation exists because the misreading has happened.
The kilogram is the odd one out
The metric system has exactly one untidy corner and this is it. The kilogram is the SI base unit of mass, and its name already contains a prefix. Prefixes cannot be stacked, so multiples and submultiples are built on the gram instead: a millionth of a kilogram is a milligram, never a microkilogram.

Since 2019 the kilogram itself has been defined by fixing the Planck constant rather than by a platinum-iridium cylinder near Paris. For everyday purposes nothing changed: the redefinition was arranged so existing mass values carried across unaltered, and the old prototype is still kept, now as an object whose mass is measured rather than assumed.
