Exactly 1,000. Divide milligrams by 1,000 to get grams, so 500 mg is 0.5 g. The step below is the same size and far more dangerous: 1,000 micrograms make a milligram, and confusing mcg with mg is a thousandfold error rather than a small one.
This converts mass to mass only. Turning milligrams into millilitres needs a concentration and is not a unit conversion.

The formula, worked line by line
One division by an exact power of ten. The prefix milli- means one thousandth by definition, so a milligram is not approximately a thousandth of a gram, it is one. The conversion cannot introduce error and no better factor exists to be discovered.

That quirk is worth knowing because it is the one place the metric system is untidy. The kilogram is the SI base unit of mass and its name already contains a prefix, so multiples are built on the gram instead. There is no such thing as a microkilogram, and anything written that way is an error.

For quantities below one milligram, micrograms are usually the safer unit precisely because they avoid a leading decimal. A number with no decimal point cannot lose one.
The kilogram, and why the ladder starts at the gram
Since 2019 the kilogram has been defined by fixing the Planck constant rather than by a metal cylinder in a vault. For everyday purposes nothing changed: the redefinition was deliberately arranged so that existing mass values carried over unaltered, to within about one part in a hundred million.

A symbol is a defined mark rather than a shortening. Changing its case changes the unit, and adding a plural s produces something that is not a symbol at all.
How to use the milligrams to grams converter
Divide by a thousand. The prefix milli- is defined as exactly one thousandth, so the conversion is exact and there is nothing to round. As arithmetic this is the simplest page on the site, which is precisely why it is worth being careful about what the number means before and after.

Micrograms are where real harm happens. Written with the Greek letter mu the symbol can be misread as an m, turning a microgram dose into a milligram one — a thousand times too much. Safety guidance therefore asks for the word microgram or the abbreviation mcg rather than the symbol, precisely because the symbol is legible until it is handwritten.
The thousandfold error is documented rather than hypothetical. Safety bodies have published incident reports of compounded medicines prepared at a thousand times the intended strength after micrograms and milligrams were mixed up, with children hospitalised as a result, and the same confusion has been flagged repeatedly over more than a decade.

The notation rules that exist because of harm
Two writing conventions in this area are not style preferences. A trailing zero after a decimal point is prohibited on medication documentation, because 5.0 mg read with the point missed becomes 50 mg. A leading zero is required, because .5 mg read the same way becomes 5 mg. Both are tenfold errors caused by a single mark.

The case rule matters more than it looks. Unit symbols are case sensitive: mg is a milligram and Mg is a megagram, which is a tonne. In a chemical context Mg is also magnesium. Writing MG or mG is not a casual abbreviation, it is a different symbol or none at all.
Where the ladder actually goes wrong
The failure is almost never the division itself. It is reading the wrong prefix off a label, working from a worksheet in one unit while a scale displays another, or converting in the wrong direction — milligrams to grams is a division, grams to milligrams a multiplication, and reversing them is a millionfold swing.

The three correct rows are the same amount. The fourth is what happens when a prefix is misread, and it is why the guidance is to write the unit out rather than to trust a symbol to survive a photocopier.
Milligrams to millilitres deserves stating separately because people ask for it constantly. Mass and volume are different quantities, and the bridge between them is density or concentration. Ten millilitres of a 5 mg per mL solution contains 50 mg. Without that concentration figure the conversion is not difficult, it is undefined.
